Use Google Forms or a similar tool to create a survey, questionnaire, or form that feeds the responses into a shared spreadsheet. Embed that form into a new page on the website you created in Week 1, and post the link to that page here in the Canvas Discussion (optionally, you can share the link to the spreadsheet, too, so we can see what the submitted data looks like.
